Enhanced Care from Curated Health is designed for individuals who value consistent, comprehensive healthcare with a team-based approach. With Enhanced Care, you’ll receive 10 in-office visits and 5 telehealth visits annually, offering flexibility and accessibility tailored to your lifestyle.
Your primary care team will include both Dr. Jennifer Peña, Board-Certified Internal Medicine Physician, and Sara Tollstrup, MSN, APRN. Dr. Peña will personally guide your care, starting with a thorough initial physical exam (the Healthy Start Visit) and developing a customized Healthy Journey care plan to address your unique health needs. Sara Tollstrup will monitor your progress, ensuring your care plan is followed and updated throughout the year. Any significant changes in your health are promptly communicated to Dr. Peña, who will step in to provide direct attention when necessary.
Enhanced Care is ideal for those who prefer or require more frequent healthcare visits, require access to specialist care from time to time, and need services like lab work and radiology. Whether you need in-person consultations or the convenience of telehealth, this plan ensures you have augmented access to high-quality primary and preventive care. Choose Enhanced Care for a seamless blend of personalized attention and team-based support, giving you the peace of mind that your health is in expert hands all year round.
Enhanced tech resources are included in Enhanced care. Members benefit from advanced health monitoring tools and access to telemedicine services, allowing for virtual consultations when an in-person visit is not feasible.
Enhanced care members enjoy a dedicated team of professionals who work to coordinate and manage your annual Healthy Journey Care Plan. Enhanced Care members typically require a more intense level of care coordination, care management and care navigation.
Leveraging the Curated Health technology suite via apps on your phone or web-based solution, you can access a Curated Health provider from most anywhere, in the world. Curated Health provides or has arranged to provide physician coverage via telehealth in all 50 states, Puerto Rico and USVI. From prescribing medications or labs to coordinating care, Curated is on standby. Even if you are traveling internationally, we can consult with you and recommend care. Please note that additional fee’s may be applicable.
Curated Health will coordinate certain transportation such as Air Ambulance services included in the Enhanced Care Membership should a covered emergency arise. Additionally, Enhanced Care members have the option to purchase Air Ambulance services that may not be covered by your insurance at special preferred rates through their Curated Health membership.
Enhanced Care members are able to purchase additional office, telehealth, visiting family member visits, doctor to doctor consults, the Curated Health Care Anywhere home diagnostic telehealth technology and nationwide mainland telehealth services for an additional fee.
Curated will provide any documentation and service records needed for members to seek reimbursement from their health insurer. However, Curated itself does not bill third party insurance for its services.
Curated Health has assembled a network of local interventional specialists. If your Curated provider refers you to a Curated Network Specialist, two non-procedural specialist visits are included with your Enhanced Care membership. Thereafter, the care recommended by the specialist may be covered by your health insurance or other financial arrangements between you and that provider.
For the benefit of its member’s, Curated Health has secured exclusive access to more than 3,300 specialists and subspecialists around the United States. Through this exclusive relationship, Curated Health can obtain expert case review and second opinions within twenty-four (24) hours of request. Occasionally, your provider may recommend consulting with a specialist to address a need that might arise. If you agree with your provider to consult with a specialist, additional fees exclusive to Enhanced members may apply.
